Sumario: Venous thromboembolism is a major cause of morbility and mortality worldwide and the appropiate management of anticoagulated patients important because of its relationship to log-term disabilities. Specialised anticoagulation facilities have improved outcomes in this group of patients by following an evidence-based approach. This revision aims to provide a decision-making algorithm when dealing with patients suffering from thrombotic disorders.
