Sumario: The aim of this work was to design a methodology for analyzing the context of the organization as a basis for safe surgical care at a clinic of third level of complexity in Bogotá. This research is of mixed type, in which the causality of adverse events were identified by three sources: 1) causes of adverse events during a year in a hospital of third level of complexity in Bogota; 2) systematic review of the most common adverse events causes in surgery worldwide; 3) analysis of perception via surveys of hospital staff and patients undergoing surgery at the hospital in question. Finally, a triangulation of information was performed to generate a methodological guide to define the context of an organization based on patient safety in surgery. It was found that the causality of adverse events is mainly due to failures in organizational processes like leadership, selection and training of personnel, communication, resource planning, standardization of procedures, verification process and implementing controls. Conclusion: The development of the organizational context for considering management systems responds to correcting organizational failures leading cause of adverse events in surgical services.
